Managing Pilonidal Sinus Pain: A Practical Guide
Pilonidal sinus pain ranges from mild discomfort to severe, debilitating pain during an acute abscess.
Pre-Treatment Pain Management
Medications:
- Paracetamol 500–1000mg every 6–8 hours
- Ibuprofen 400mg every 8 hours with food
- Diclofenac gel applied topically
Non-Pharmacological:
- Warm sitz baths (15–20 minutes, 2–3 times daily) — most effective non-drug measure
- Donut cushion — reduces pressure on the natal cleft
- Ice pack (wrapped in cloth) — 10–15 minutes every 2–3 hours for acute abscess
Post-Operative Pain Management (After Laser Treatment)
Day 1–3: Paracetamol 1000mg every 6 hours + Ibuprofen 400mg every 8 hours. Most patients rate pain as 2–3/10.
Day 4–7: Paracetamol as needed. Pain should be decreasing daily.
Week 2: Most patients are pain-free at rest.
Post-Operative Pain (After Open Surgery)
Day 1–3: IV paracetamol + IV diclofenac. Pain typically 4–6/10 on Day 1.
Week 1–2: Paracetamol 1000mg every 6 hours + Diclofenac 50mg twice daily.
When Pain Indicates a Problem
Contact the clinic immediately if pain is increasing after Day 3, accompanied by fever, or the wound looks more red or swollen.
