What Is FiLaC Laser Treatment?

FiLaC stands for Fistula-tract Laser Closure. Originally developed for anal fistulas, the technique has been adapted for pilonidal sinus disease (also called SiLaC — Sinus-tract Laser Closure). The procedure uses a radially emitting laser fiber to ablate the epithelial lining of the sinus tract from the inside out.

How the Procedure Works

Step 1: Preparation
- The patient is positioned prone (face down)
- Local anesthesia or spinal anesthesia is administered
- The sinus tract is cleaned and irrigated

Step 2: Laser Fiber Insertion
- A thin laser fiber (typically 360° radially emitting) is inserted through the sinus opening
- The fiber is advanced to the deepest point of the tract

Step 3: Laser Ablation
- The laser is activated as the fiber is slowly withdrawn
- The laser energy destroys the sinus lining at 360°
- The tract collapses and seals from the inside

Step 4: Closure
- The external opening is closed with a single suture or left to heal
- The procedure takes 20-40 minutes
